The City of Middletown Health Department (CMHD) is now scheduling Moderna Covid19 vaccinations online at https://gettheshot.coronavirus.ohio.gov. Clinic locations, dates and times will be available for you to select as well as from other providers such as Kroger’s, CVS, etc.

If you do not see an appointment that works for your schedule, please continue to watch for additional clinics with CMHD. Clinics will be held at the Middletown High School on the 4th Sunday of April & May. The clinics are WALK-THRU and you will need to be able to walk and stand in line for extended periods. Drive-thru clinics are being offered weekly by Butler County Health Department and City of Hamilton Health Department. Please visit their websites for available dates and times. We are currently scheduling appointments for Sunday, April 25th

  1. If you are scheduling your 2nd dose of Moderna vaccine, your 1st dose needs to have been given on or before March 28, 2021.
  2. Please remember to bring your CDC COVID-19 Vaccination Record Card with you when receiving your 2nd vaccination. If you do not bring the record with you, this WILL increase your wait time while we verify your 1st vaccination information.
